Is it true that if you are taking an SSRI but are not producing enough serotonin, that the SSRI will not have enough to work with? Do you also need to raise your serotonin level in some way?????

Not completely. SSRIs increase the extracellular level of the neurotransmitter serotonin by inhibiting its reuptake into the presynaptic cell, helping to overcome depression, anxiety, sleeplessness, and sometimes even premature ejaculation. So you don't have to raise your serotonin in other way.

I am bipolar and severely depressed after a recent cancer (choroidal melanoma) diagnosis. Currently I am on Lithium 450, paxil 60, doxepin 10, ativan 1.0 and still severely deppressed, any suggestions ?

So you need not only anti-depressants, you need special psychiatric help including talks, and support from your friends and relatives. Maybe you should join anti-cancer community to realise that many people have overcome cancer and it is possible to live with it. Continue paxil and other prescribed medicines. Also this treatment does not start to act at once, you need to wait 2-3 weeks for best effect. Good luck!
